If you can offer programming in the counties she notes are in Region 12, please review the 5 criteria and reach out to Berta with your information.

  Berta Rivera, Regional Health Connector for Region 12 (Pitkin, Eagle, Garfield and Summit counties) manages an online resource directory for region 12 and wanted to offer the opportunity to add your programs to the online resource. Please note the information she requests and the example she provided.

  Resources Archive - West Mountain Regional Health Alliance See below for the necessary information needed, if you have any questions please reach out. 

  1. The resource directory focuses on free, low cost or reduced resources. Please only list your resources that fall under this criteria.
  2. If you have any resources that are intended to be utilized by regular clients/patients and not by walk-ins, please refrain from listing these resources.
  3. Please complete the resource questionnaire. There are seven resource information categories which are needed.

  3. 1. Services: Summarized list of the resources your organization offers (checks off points 1 and 2). Ex: Food, language (Spanish), volunteering, financial, internship, housing, etc. Resources are not limited to specific areas.
     2. Cost: What are the expenses to utilize the resources?
     3. Eligibility: Who can access the resources and what steps are needed?
     4. Hours: When are the resources accessible?
     5. Address: Where are the resources? If resources are offered online (for example) where is your organization located?
     6. Phone: Where to call to access or get information on the resources?
     7. Website: Where to apply or get information on the resources? 

                           
  

  

  
  

  4. If you have programs/resources that have different category responses (cost, eligibility etc.) please provide separate resource questionnaires.
  5. If your organization has a resource guide/directory, please provide the link to it so it can be added to the resource guides/directories tab.
